All human beings are capable of distinguishing the real from the unreal (imaginary). But there are a few who refuse to recognize this distinction.

Anu (Bipasha Basu) a fine art student, from a happy and affluent family experiences a tragedy. On September 11, 2001 terrorists attached New York's World Trade Center, killing Anu's elder sister and her brother-in-law, and the impact of the tragedy shook her badly and she had a mental set back. To help her overcome this problem her parents get her engaged to Arpit Oberoi (Priyanshu Chatterjee) a well known Ad-Filmmaker, who is later commissioned for an international Ad Film project and leaves for the U.S. Meanwhile Anu meets Aman (John Abraham) who is member of an anti-terrorist organization. Enamored by Aman's purpose in life Anu falls in love with him. When it is time for marriage with Arpit she refuses to marry him and cuts her wrists in protests. Anu survives and it is revealed that she suffers from Schizophrenia. 

When and how and whether at all Anu overcomes this dreaded disease & how she deals with the two men in her life form the interesting conflict of MADHOSHI.
